To complete the online application, you will need your group clients’ contact information including tax ID, effective dates, eligibility, core and voluntary benefits being pre-taxed, and any affiliate company information.
When you submit your online application, you will designate the email address where the Documents will be sent. You can send the Documents to multiple email addresses by separating them with a comma, so if you’d like, you can send them directly to your client, broker and yourself. The designated email address will receive a customized Premium Only Plan Document, Summary Plan Description and Adoption Resolution within 15 minutes of the application submission.
